Thank you for your interest in joining the effort to connect New Yorkers in need to the information and services they deserve.
This form will help F.Y. Eye evaluate if your organization's location is well suited for our mission to
disseminate public service announcements (PSAs) that generate awareness of not-for-profit organizations and
their programs as well as important social, civic service and educational messages.

What services does your organization provide?
Approximately how many people visit your location each month? If possible, please include information on total visitors and unique visitors to your location.
Please provide the ethnic breakdown of the community that will be exposed to the screens. Please be as detailed as possible and include percentages.
What is the estimated percentage of unique visitors who fall below the poverty line? (please use the New York City Poverty Line).
Please indicate the estimated percentages of your constituents by age? (Children, Teen, Adult, Elderly, Families).
