As a New Yorker, you have the right to appoint someone you trust to make medical decisions for you if you become sick or injured and can't speak for yourself. By appointing someone you trust, and who knows what matters to you, your wishes can be known and honored. A new, free guide — A New Yorker's Guide to End of Life Planning, Options & Rights — provides everything you need to appoint your decision-maker, as well as comprehensive information on other types of planning and care. About the Organization

End of Life Choices New York is a non-profit organization with the mission to improve end of life care, expand end of life options, and promote healthcare equity at the end of life. They offer education and resources for New Yorkers and healthcare professionals; advocate for improved and expanded patient rights and options; and provide direct guidance for individuals and families.
