
JCCA’s Health Home PSA highlights a person‑centered approach to family care in New York City, starting with a simple question: “Is your child struggling?” It introduces a dedicated care coordinator who manages your child's physical, mental, and social service needs—scheduling appointments, supporting treatment plans, and offering condition‑specific guidance. Coordinators also connect families to community resources and stable housing, help with public benefits enrollment and renewals, and arrange hospital follow‑up and discharge planning.
